We’ll need following information to set your account up:


(1)  Billing information:


- Company name


- Company address


- Contact name including email and phone number


- Method of payment


(2)  Media shipment information:


- Ship to address for media (No PO Boxes, please)


- Recipent's to name


(3)  Reporting information:


- Report recipent's name, email address, and cell phone number


- Method of delivery (email, fax, or hard copy)


Galson's 3 and 1 Agreement

You'll find a link to Galson’s 3 and 1 Agreement located in our Resource box.  You’ll need to download and print the "Rental, Pump Loan, Free Badge Agreement," chose from one or more of our client-friendly programs, sign and fax the form to Client Services at (315) 437-0571. If you'd like to send the form via email, please remit to clientservices@galsonlabs.com.  Once we receive the above information, your account will be set up and we'll e-mail your client number and password to you.  If you need immediate account set up contact Client Services at (888) 432-5227.
